Client Education Materials
Family PACT providers have access to a wide range of easy-to-read and culturally appropriate client education materials.
To order print materials:
1. Download and complete the order form.
2. Email the completed form to OFPWarehouse@dhcs.ca.gov (preferred option).
OR
3. Fax the completed form to (916) 440-5656.
Orders by phone are not accepted.
